Our mission is to transform the way chronic disease is addressed.
We are getting systemic drivers of disease on the agenda, putting the community in the centre of the problem-solving process and facilitating cross-sector collaboration.
Managing chronic disease is a costly business. Australia spends $44bn annually on chronic disease management, and yet only $89 per person on prevention – despite researtch showing that up to 70% of health outcomes are driven by our lifestyle choices.
We are putting the systemic drivers of disease on the agenda, ensuring that local communities are at the centre of the problem-solving process and shifiting the focus from symptoms to causes.
We build coordinated movements around a specific problem or local communities. Using a systems thinking approach, we co-design and mentor community-led solutions that better leverage existing infrastructure and resources to create connected and responsive health ecosystems.
We’re on the path to learning how to be an excellent national backbone organisation, supporting effective collective impact solutions.

Refugees, people seeking asylum and disadvantaged migrants have limited access to the health care they need to be well-integrated in our communities. They often have complex health problems and fall through the gaps due to language and cultural barriers. In partnership with the local refugee community and Brisbane refugee health and social services, we are creating collective impact solutions to their most pressing problems.
